Enrollment

To get started, please click the DSS Application Form link and complete the application. Once you have finished the form, fill out the General Information at the bottom and click submit. After both the DSS Application Form and the general information section are completed, the process will begin.

Please complete the following steps:

​

a. Email a copy of a photo ID for all individuals listed for drop-off and pickup to dr.saxonkingskidsgwd@gmail.com.
b. Bring a copy of the child’s shot record and birth certificate.
c. Please be prepared to submit the deposit in order to be added to the waitlist.

 

All remaining signatures and any additional discussions can take place while we wait for the arrival of our little one.
